(All submissions confidential)
This form is for reporting a sexual assault anonymously to the Buffalo State University Police, in compliance with the Federal Student Right to Know and Campus Security Act, to assist with the compilation of statistical records for assaults that occur on the campus. Filing this form will not result in an investigation unless requested. A person who has been assaulted may fill out and submit this form himself or herself or may ask a third party (such as a friend or a counselor) to do so. Without compromising the victim’s confidentiality, this anonymous report can alert the campus to the fact that an assault has occurred and can assist the Buffalo State University Police in detecting patterns and preventing future assaults.
The State University of New York and Buffalo State University want victims to get the information and support they need regardless of whether they would like to move forward with a report of sexual violence to campus officials or to police. Links to the sexual assault victim’s bill of rights and a Notice to Victims of Sexual Assault is available within this report.
If the victim needs immediate assistance, please call University Police at (716) 878-6333.
Campus personnel, other than those with significant counseling responsibilities (such as licensed professionals or certified counselors), must file a report when informed by a student of a sexual assault, to maintain campus compliance with the Federal Crime Awareness Act.
